Your First Visit to Premier Urology
For your first visit to Premier Urology, please bring the following items with you:
- A valid state ID
- Your insurance card (if you have multiple insurance, please bring all cards)
- Appropriate copay
- Your completed new patient form
Important Information:
- Please be ready to give a urine sample
- Don’t forget to fill out your new patient paperwork in advance – failure to bring your paperwork will result in your appointment being rescheduled
